Students who wish to enroll in an intership or practicum experience should begin by clicking on the link to the right. This will open a PDF file containing paperwork required for creating an intership or practicum course.

• Waiver of Liability form for Internships and Practicum Participants

Students will need to obtain a Permission to Enroll Card from the Exercise and Sport Sciences Department Secretary after obtaining signatures on the Contract.
