Academic Forms

Various forms required by the Academic Affairs office are accessible from this page. You may print a form, fill it out and turn it in at the Academic Affairs office or when appropriate, the form may be filled out on-line and sent electronically.  All electronic forms should be sent to Kim in the Academic Affairs Office at KSMeyer@manchester.edu
