The forms below are mandatory for all fall sport athletes (volleyball, football, soccer, tennis, softball, cross country and baseball) to fill out and either send, fax or bring to campus before August 1st, 2008. 

Please read all attachments which will clarify and explain all forms, insurance and billing information for the athletic training department.  all forms should be completed and sent to the below address before the deadline in order to begin participation in athletics on time.  All first year student athletes will also have to complete a mandatory physical for the athletic department before participation is allowed. 

This physical will be scheduled through the athletic training department and performed before the season starts on campus by our team physicians at no cost.
