What does it cost to see the nurse?
There is no charge for an evaluation by the nurse.

What are Health Services hours to see the nurse?
The nurse will see patients by appointment only. Please call 982-5306 to make an appointment.
How do I contact Health Services during summer months?
The office is closed during June and July; however e-mails are typically checked once a week. If you need an immediate response you may contact Student Development at 260-982-5052.
What if I am ill after hours during the academic year?
If you are ill after office hours you may contact the Student Health Assistant on call at 260-901-3123 or call Campus Security at ext. 5999 or 911 in the event of an emergency.
What is a Student Health Assistant (SHA)?
SHAs work under the supervision of the Director of Health Services and are considered part of the Student Development student staff. SHAs are on call from 5 p.m. until 8 a.m. on weekdays and from 5 p.m. Friday until 8 a.m. Monday on weekends. SHAs understand, follow, and sign the statement of confidentiality upon beginning employment with Manchester University Health Services.
How do I contact a Student Health Assistant?
The SHAs are on call from 5 p.m. to 8 a.m. during the week and all weekend when classes are in session. They can be contacted by calling 260-901-3123.
Is my office visit confidential?
Students who are 18 years of age or older are considered adults and have the legal right to medical confidentiality. We do not and will not release any information to anyone about you without your written or verbal consent to do so.
Can my parents have access to my medical records?
They can only have access to your medical records if you give written or verbal consent.

I am going abroad for a January term class or in Brethren Colleges Abroad (BCA). What immunizations do I need?
Contact Health Services as soon as you decide to travel. Necessary/required vaccinations vary depending upon travel location. Some vaccines are in a series and take time to complete.
Do you give allergy injections?
We do not give allergy injections. You may contact Manchester Clinic at 260-982-2102 or Walnut Street Clinic at 260-982-1984.

Do I have to complete a health form?
All new incoming students to Manchester University must complete a health form regardless if an athlete or not.
If I am an athlete do I still have to have a health form completed?
Yes, you will still need to complete a health form.
If I am a transfer student do I still have to have a health form completed?
Transfer students need to complete a health form.

Health forms are due by Aug. 1.
What if my physical date is scheduled after Aug. 1?
You need to notify Health Services at 260-982-5306 by Aug. 1.
Do you offer any immunizations?
We do offer immunizations at Health Services for a fee.

The health form is only required for first time students. Once we have the form on file you do not need to complete it again.
Do I have to have a meningitis vaccine?
The meningitis vaccine is required for all students.
Will my insurance pay for the vaccines or medicine given by the nurse?
Vaccines or medicine given by the nurse will be billed to your tuition account or paid for at the time of service. A claim can be filed by you or your parents with your private insurance carrier. The Bollinger insurance will not cover the vaccines but may cover other medications.
Is it mandatory to have the Bollinger insurance?
All students are required to be covered by insurance. If you have medical insurance through your parents or on your own you do not have to have the Bollinger insurance. A waiver card MUST be completed.
What is the electronic waiver card and when is it due?
The waiver card informs the University that you have insurance coverage and are declining the Bollinger coverage. The waiver card must be completed online by August 1st. A copy of your insurance card(front and back) needs to be sent to Health Services as well. Here's a link to the electronic waiver form: form
What happens if I don’t turn in a waiver card?
If you do not turn in the waiver card then a charge will be placed on your tuition account for the Bollinger Insurance. There will be no refunds after Aug. 31.

What if I have specific questions about the insurance?
You can call Bollinger Inc. at 866-267-0092 and ask to be connected to the department that handles Manchester University student insurance.
I have my own insurance and don’t need Bollinger insurance. What do I need to do?
You must complete the waiver card and provide Health Services with a copy of your insurance card (front and back) by Aug. 10.
How do I get an insurance ID card?
You can download a copy of the ID card from the Bollinger website.
Where do I send claim forms?
You can turn the claim forms into the Health Services office as they are submitted weekly. Or you send the claim forms to Bollinger Insurance at P.O. BOX 727, Short Hills, NJ 07078-0727. This address is on the claim form.
What do I do if I am ill and not on campus since I have Bollinger insurance?
If you are not on campus, seek medical treatment, pay the bill and then file a claim with Bollinger.
