Information for New Students

Students planning to come to Manchester must complete the following paperwork:

• Health History Form – Due August 1
  Includes physical examination and record of immunizations. Find more information here.
  
  
• Insurance Waiver/Enrollment Form – Due Aug. 1

All students are required to be covered by health insurance. If you don't have pre-existing coverage, you will be enrolled into the University's student health insurance plan (Bollinger Insurance). You may waive coverage if you do not wish to be billed for the University's plan. You must complete the Waiver/Enrollment Form, whether you plan to purchase the University's plan or remain on an existing one. Find more information here.
