Health Physical Forms

Like all other students, athletic training students are required to submit a health history and  physical form to health services before beginning classes at Manchester University. Completed forms are reviewed by the program director for any medical/health issues which could impact the health and safety of the athletic training students. Copies of the forms are maintained in the records kept by the program director and the academic clinical education coordinator.
