The Office of Student Affairs works closely with University Health Services on the North Manchester campus to coordinate health and immunization records. Students can utilize a variety of health care facilities in the Fort Wayne area for any health care needs.

Health and Immunization Record

All students are required to have completed the following; personal health history, physical exam, and immunization record. These components are provided to students prior to enrollment and must be on file prior to the start of fall semester classes.

Health Insurance          

All pharmacy students are required to have an active health insurance plan in effect prior to the start of experiential rotations. The University does not offer an option for coverage. Students are encouraged to compare insurance options; a common resource is the Health Insurance Marketplace.
