Health Requirements for Incoming Students

All medical history, immunization, and student-athlete information needs to be completed by July 30th in order to access your dorm room and participate in fall athletics. 

I. Medical History:

(Required for all Undergraduate and Graduate students)

All undergraduate and graduate first-year and transfer students are required to submit a medical history. Medical information is strictly confidential and is used to provide the appropriate care for the student when needed. You can complete your medical history form by accessing your Med+Proctor account.

II. Immunizations:

(Required for all Undergraduate and Graduate students)

Manchester University requires all entering students to present evidence of immunity to certain diseases before they may arrive on campus. Students whose religious beliefs conflict must fill out the religious exemption formlocated on your Med+Proctor .


Required immunizations are:

MMR-Measles, Mumps, Rubella (series of two)

Meningococcal or Menactra or MCV4- (series of two)

Meningococcal B (series of two)

TDap/tetanus/diphtheria (within the past 10 years)

For International, Advanced Nursing, Graduate and Doctoral students ONLY: 

Tuberculosis screening test or QuantiFERON Gold Blood test (must be less than 12 months old)

Recommended but not required immunizations are:

Covid-19 Vaccination

Flu Vaccination (REQUIRED FOR PharmD & ABSN Students)



III. Emergency Medical Contact Information:

(Required for all Undergraduate and graduate students)

All incoming students are required to complete the Emergency Medical Contact Information using their MU login information. The form can be completed on our ChetNet site at the following link:

IV. Physical Examination

(A physical exam is required for students participating in intercollegiate sports only)

Students planning to participate in intercollegiate sports must have a physical within the past 6-months from the start of the academic school year for those students enrolling in the fall. For those students enrolling in the spring, sport physicals must be within 6-months from the start of the spring semester. If you do not plan on participating in intercollegiate athletics there is no need to get a physical examination.

V. Athletic Forms:

(Required for student athletes participating in intercollegiate sports)

VI. Athletic Insurance:

It is required to have health insurance in order to be eligible to participate in athletics. You can complete proof of this requirement by uploading a copy of your insurance card to Sportsware. The university only provides health insurance to our undergraduate international students.
