Posted Feb. 15 - Teresa DeLellis, assistant professor of pharmacy practice, will co-present on “Effect of a Multi-Setting, Interprofessional TOC Pathway on 30-day Readmissions in Older Adult Heart Failure Patients” at the American Geriatrics Society Annual Meeting in Portland, Ore., May 2-4, 2019.  At the same conference, she will co-present on “Effect of Geriatric Pharmacist TOC Interventions on Heart Failure Readmission Rates.”

At the Annual Meeting of the American Association of Colleges of Pharmacy in July, she will co-present on “One Size Does Not Fit All: Customized Approaches to Designing, Operationalizing, and Assessing the Co-Curriculum” and  “Innovative Ways of Keeping It ‘Old School.’” The AACP will meet in Chicago.
